Cancellation/No-Show Policy
We care deeply about our patients, and want to be as efficient and effective as possible. To this end, we follow a strict policy regarding cancellations and no-shows. This policy protects both our patient's time as well as our ability to see as many patients as possible.
We understand that sometimes there are emergencies. If this occurs, notify us as soon as possible. We may request additional documentation of your emergency.
Cancellation/No-Show Policy – New Patients
When you schedule an initial visit, you will be expected to pay the full appointment fee.
If you do not attend the initial visit or cancel with less than 24 business hours notice, you have until the following business day to contact the office and reschedule your appointment.
If you reschedule within 1 business day and attend the appointment, the full payment will be applied to your visit. (No cancellation penalty.)
If you do not reschedule within 1 business day, you forfeit a $100 cancellation fee. If you have paid in advance for your appointment, the remainder will be refunded to you within approximately 2-3 weeks.
If you also fail to attend your rescheduled appointment or cancel it with less than 24 hours notice, you forfeit your entire payment. No refund will be made and you will not be rescheduled.
Any appointment reminders are a courtesy. The patient bears full responsibility for remembering the appointment.
Cancellation/No-Show Policy – Established Patients
If you do not attend a follow-up visit or cancel with less than 24 business hours notice, you will be charged a $50 cancellation fee.
After you have paid the cancellation fee, you will be permitted to reschedule.
If three or more no-shows or late cancellations occur within a 12-month period, you may be discharged from the practice.
Your First Visit
Welcome to Wellspring! If you've made your first appointment then we're excited to welcome you into the practice! Here's what to expect leading up to and through your first visit:
One Week Before: Make childcare arrangements for any children who will not be seen at the appointment. We ask that children not be left unattended in the waiting room, but nursing infants are always welcome to stay with Mom. Your initial appointment will likely be 60-90 minutes.
One Day Before: Complete all forms, at home, if possible. Gather your current medication bottles to bring to your appointment . Please do not bring in any loose medications (not in the prescription bottles). Prepare a list of any supplements or over-the-counter medications that you currently take on a regular or as-needed basis.
When You Arrive: The office is open for appointments only, so do not come earlier than instructed. When you arrive at Wellspring, have a seat and relax. If you have completed the paperwork provided to you in advance of the appointment, you will be asked only to complete a one-page screening tool. If you've missed anything, we'll ask you to fill it out then. If you need to complete all of the paperwork at the office, please allow 30 minutes and notify us in advance. Paperwork must be completed before the appointment, so that we can spend your full appointment time focusing on you.
During Your First Appointment: This is a relaxed environment. There is no medical table or psychiatrist's couch. You'll meet with your provider in a comfortable office, and begin by simply having a conversation. The more open and honest you can be with your provider, the easier it will be to develop your treatment plan. However, you are never required to talk about anything that you don’t feel ready to disclose. At Wellspring, we value the perspective of the patient and family members as a key part of the treatment process. Your questions and concerns matter. Don't worry if your provider is taking notes during your appointment; this is to ensure accuracy in your care.
After Your Appointment: If you need a follow-up appointment, we'll schedule it while you’re in the office so that you don't need to worry about it later. We'll see you next time!
